A Diagnostic Assessment is an intensive clinical and functional face-to-face evaluation of an individual's mental health, developmental disability, or substance abuse condition. The information is reviewed thoroughly and signed off by both the therapist and Clinical/Medical Director, providing a complete Diagnostic/Assessment report including clinical recommendations regarding whether the individual meets the target population criteria. This assessment also includes an order for Enhanced Benefit services that provides the basis for the development of an initial Person Centered Plan (PCP) .
For substance abuse identified assessments, the designated diagnostic tool (SUDDS IV, ASM, SASSI) required by DHHS for specific substance abuse target populations are conducted and included in the assessment.
A Diagnostic Assessment is signed and dated by the therapist and Clinical/Medical Director and served as the initial order for services included in the PCP. Upon completion, the PCP will be sent to the LME for administrative review and authorization of services under the guidlines of the LME.
Hi, my name is Demi and my experience with C3 has been amazing. The therapists have been awesome with teaching me coping skills and correct me when I was wrong. The best therapist that I loved was Takisha, AKA Takiki. Me and her connected the most because she is funny, she knows where I'm coming from and I like her sense of style. I love Amanda because she gets me and has nice taste in music. Gerald is a nice guy. Thank you!
